Community members may apply for financial aid assistance provided they completed the application below in its entirety and certify their eligibility.

Any misrepresentation or fraudulent information provided may result in disqualification from current and future assistance. Furthermore, by submitting this application, you hereby release and hold harmless ISOD, its staff, employees, board, volunteers, and affiliates from any and all liability, including but not limited to financial, legal, or tax-related consequences arising from the receipt or denial of aid.
